When you create a survey, you can specify the following components to include with the survey:
Custom Logo and Text & Background Colors - If you want to include your own logo on the survey, create a .GIF or .JPG image that measures about 60 x 60 pixels. When you create a survey, you will be able to attach this image to your survey. In addition, you can specify the color of the survey background and text. Make sure you create a logo that will look good on the background color you select, and choose a text color that contrasts with the background so respondents can read the survey items. Welcome Letter - The optional "Welcome Letter" will introduce survey respondents to the survey, thank them for taking the time to complete it, and exlpain to them the reasons the survey is being conducted. Before you start creating the survey, you should compose this letter in your word processor and check it over for spelling and grammar. When you create the survey, you can copy and paste the letter into the Survey Administrator.
Survey Instructions - If you have special instructions that you want to pass along to survey respondents, you can list them on the survey instructions page. Such instructions might urge respondents to include specific comments or define terms used in the survey items. Default instructions on how to complete the survey will be included automatically.

Specifying an Exit Page - If you want to route respondents to a page on your web site once they complete the survey, you can specify the URL of this page when you create a survey.
Respondent Personal Info - You can specify several fields of personal info, such as name, email address, and phone number, that respondents must enter when completing the survey.
Easy Access Link - To make it easy for your respondents to access your survey, you can create a special link or URL that will take people directly into your survey.
